Dear Parents
Let me inform you how the health system in the Czech Republic works for children.
In our country there is a medical system of preventive checks. To the age of 1 year, every 2 months. For older children it is each odd year during your stay with the child here. We are not here only for various statements, we would like to heal and support the child with the whole family.
Please, in contact with us write the name of the child, date of birth, your address and phone number.
To the new patients we send in attachment the ENTRANCE INQUIRY, which, please, fill in and resend to us as a PDF text.
Prepare for us all documents in paper form: INSURANCE, VISA or PASSPORT, and VACCINATION LIST from home country.
